- Your site meets one or more of the disqualification criteria.
- You did not read the criteria first.
- You don't want to answer all the questions in the form below.

| How do I apply? |
- Fill out the complete form below if you want to apply for the award.
- Please mention your name, your age, your site name, the site URL and describe why your site should receive the award.
- Click on "Submit Form" when you are ready to send.

| What about my data? |
- The information you provide will be given to no other person or organisaton but to Silver Dragon Breath.
- Your information will only be used by Silver Dragon Breath to see if you are not under 13 years old, to review your site and to respond to you by email.
- Your email address will only by used to notify you once within three weeks in case your site wins the award.
- If you win the award, your name, your site-name and site-url will be places on the winners list.

| When will I be notified? |
- Only sites that are going to be rewarded, will be notified by email within three weeks after applying.
- Sites that aren't going to be rewarded, will be not be notified.
